The Mainstay Foundation is launching a dedicated medical mission to underserved villages across Africa to provide essential medical care, educate local communities, and support ongoing humanitarian efforts. We are inviting compassionate and qualified dentists and medical professionals – like you – to apply.

This initiative is designed to deliver essential healthcare, strengthen public health education, and support long-term humanitarian efforts on the ground. You’ll be part of a team bringing hands-on care to communities that are often overlooked and under-resourced.

Mission Overview

• Over the course of the mission, medical professionals and dentists will visit multiple villages, offering hands-on care and health services to underserved populations.

Mission Objectives

• Deliver Medical Services: Provide direct healthcare support to communities in need, addressing both urgent and long-term health concerns.

• Build Sustainable Connections: Establish meaningful relationships with communities and fellow dentists and medical professionals to encourage continued support beyond the mission.

• Advance Global Health Awareness: Participate in and document experiences tied to the Clean Water Initiative, highlighting its critical role in improving public health outcomes.

WAIVER OF LIABILITY

I acknowledge that I wish to participate in The Humanitarian trip in Tanzania that will take place in 2025 or 2026. In consideration for inclusion as a participant in this trip, I hereby release, waive, discharge, and covenant not to sue the Mainstay Foundation and any of the organizers or participants, from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, up to and including death, that may be sustained by me, or to any property belonging to me, while participating in such activity, regardless of whether such liability arises in tort, contract, strict liability, or otherwise, to the fullest extent allowed by law.

I understand that The Mainstay Foundation does not provide insurance to cover any loss or damage to my personal property and The Mainstay Foundation strongly recommends I carry my own travel, health medical and property insurance for potential loss or damage whilst on this trip.

SPECIAL MEDICAL REQUIREMENTS

Participants will be responsible for bringing their own medication for allergies or other medical conditions.

SAFETY

In case of injury or accident or illness during the trip, the participant will be taken to a hospital. If their health is determined to be serious, or such that in the view of the organizers they will be unable to continue the trip, the participant may be sent back home.

MEDIA CONSENT

I, the undersigned, hereby consent to the use of my photograph or likeness in any publication, videotape, pamphlet or promotion by The Mainstay Foundation or other agencies which are promoting or furthering the mission of The Mainstay Foundation.

I understand that I will not receive separate compensation or consideration from The Mainstay Foundation or anyone else for the permission granted in this Consent nor for the actual publication or use of my photograph or likeness.

By signing this Consent, I understand I am releasing The Mainstay Foundation from any and all liability that may occur as a direct or indirect result of my photograph, the release of my identity, or the public relations materials, including but not limited to the use of any quotations.
